What you need to know about an immigration medical exam - Form I-693
Designated Civil Surgeons
Immigration medical examinations conducted inside the United States must be performed by a civil surgeon who has been designated by USCIS. PreventivePLUS is a U.S. Citizen and Immigration Services (USCIS) designated civil surgeon and is authorized to perform USCIS medical examinations.
Form I-693 is used to report results of a medical examination to USCIS.
Bring the following to your medical exam:

Form I-693, Report of Medical Examination and Vaccination Record

Government-issued photo identification, such as a valid passport or driver’s license. If you are 14 years old or younger, bring identification that shows your name, date and place of birth and parent’s full name. Possible forms of identification include your birth certificate (with an English translation) or an affidavit.

Vaccination or immunization record (such as DT, DTP, DTaP, Td, Tdap, OPV, IPV, MMR, Hib, hepatitis A, hepatitis B, varicella, pneumococcal influenza, rotavirus and meningococcal disease)

Medical insurance card (Only for blood work)

Payment (check with PreventivePLUS about acceptable forms of payment).

NOTE: 10/19/17. Starting Jan. 2, 2018, civil surgeons must use the 10/19/17 edition of Form I-693. USCIS will not accept the 02/07/17 version (or any previous editions) that a civil surgeon signed and dated on or after Jan. 2, 2018. You can find the edition date at the bottom of the page on the form and instructions.
